An overly wide, square or rectangular face can be due to a hypertrophic masseter muscle, and is often seen among Asians.
A study (Classification of Masseter Hypertrophy for Tailored Botulinum Toxin Type A Treatment) published in the Journal of Plastic and Reconstructive Surgery) has allowed us to classify masseter hypertrophy. The study defines five degrees of (minimal, mono, double, triple and excessive) and three degrees of thickness (<10 mm, 10 -14 mm, > 14 mm).
Why do we get an overly wide lower face?
Excessive width of the lower face can be caused by three factors:
– The bone structure.
– The presence of fat.
– A large masseter muscle.
Reducing masseter volume and streamlining the face. How can we get a v-shaped face?
In many countries botox is used to refine the face and get the V-shape.
Botulinum toxin injections reduce the strength, and volume of the masseter muscle.
Botox injections are recommended if:
– The masseter muscle is responsible for the width of the face.
– The patient wants to streamline his or her face.

There are no side effects.
The practitioner must have mastered the technique. A faulty technique can lead to a falling upper lip, and edge of lip, which leads to dribbling, and an inability to keep food and liquid in the mouth, as well as a faulty smile.
Results are gradual and will appear in the week following the injection, the lower face will appear slimmer, giving a v shape to the face.
